How to Apply

A family that wishes to reside in public housing must apply for admission to the program. Applications are available at the management office located at 315 Scott Avenue, Monday through Friday from 9:00 am to 3:30 pm. Families may also request an application be mailed to them by calling the office at 620.223.4570.

The PHA will provide reasonable accommodations as needed for persons with disabilities to make the application process fully accessible. The PHA will also take reasonable steps to ensure meaningful access to the program and activities by persons with limited English proficiency.

Once an application is returned, processing typically takes 10-14 business days. Once verification is complete, you will be notified by phone and/or mail regarding your eligibility or the reason for denial.

Documents that will need to be returned with the completed application:

  • Driver's License or Photo ID for each adult (18+)
  • Social Security Cards for all household members
  • Birth Certificates for all household members
  • Proof of Enrollment for any Education
  • Proof of Income - provide documentation for all sources of income, including:
    • Social Security / SSI award letters
    • Pay stubs from employers
    • Child support
    • TANF
    • Any other income or assistance

Asset information:

  • Bank information (copy of most recent bank statement for all accounts)
  • Investments such as IRA, trusts, CD's, pensions, etc.
  • Any other requested information by the housing authority

Housing History - Last 5 Years
The application must include the past 5 years of housing history, including your current address. If you rented, you must provide complete contact information for each landlord or property manager.

Incomplete housing history will delay processing. Your application will not move forward until all information is provided.

Failure to Provide Information
Refusing or failing to complete required forms or provide requested documentation may result in denial of your application.
